Vanderloo Family Medicine offers affordable Direct Primary Care (DPC) memberships for individuals, families, and small businesses. Vanderloo Family Medicine provides comprehensive and compassionate care to our patients. What does my Direct Primary Care (DPC) membership include?
unlimited office visits
unlimited telemedicine/telehealth appointments
unlimited texting, video conferencing, emails, phone calls directly with Dr. Vanderloo
Here are a few of the many primary care services Dr. Vanderloo can provide:
management of chronic conditions, such as diabetes, high blood pressure, high cholesterol
treatment of common ailments, such as common colds, ear infections, strep throat, flu, skin rashes, acne, warts
skin biopsies, as well as removal and/or drainage of cysts
stitching or suturing of minor cuts and wounds
education on weight management, exercise, and healthy lifestyle choices, including BMI measurement
management of mental health illnesses, such as grief, depression, anxiety, and other mood disorders
provision of yearly wellness exams and physicals, including sports physicals
assistance with tobacco cessation including chewing tobacco, cigarettes, e-cigarettes, and vaping
and much more!
What does my Direct Primary Care (DPC) membership cost?
Adult Membership, for ages 19+: $65.
Child Membership, for ages 18 and under: $35 (must have at least one adult member enrolled).
Family Membership, includes 2 adults and 2 children: $165.
Additional children may be added: $20 each child, up to a family maximum of $205.
Student Membership, for full-time students only, proof of enrollment required, ages 18+: $45. Please contact our office to verify school status and receive your special registration code.
Small Business Membership, $60 per employee. Please call our office to discuss your business needs so we can best meet your goals.
Please note: There is a one-time Registration Fee in the amount of $25 per patient due upon enrollment, except for Medicare patients. The patient will be informed of any fees or charges that are not included in the membership (i.e. fees for non-covered services, such as labs, medications, procedures, etc.), and payment shall be due at the time of service.
Our Policy
Scheduling an Appointment:
Please schedule your appointment before arriving.
Please wear a mask for sick-visits and practice physical distancing when needed.
We schedule only one patient at a time to decrease your wait-time and to help maintain physical distance between patients.
Cancelling or Rescheduling of an Appointment:
If you need to cancel an appointment or reschedule, please contact us at least 24 hours ahead of time, when possible.
Failure to cancel or reschedule an appointment will result in a $65 fee billed to your account.
After three missed appointments, a patient may be discharged from the practice.
If you need to cancel after business-hours, please send us a message via the “Get in Touch” button below.
If a patient is 15 minutes late past their scheduled appointment time, we will reschedule your appointment for another day/time.

Insurance Information:
If you do NOT have health insurance: Patients without health insurance pay the membership fee each month on the same billing date.
If you do have health insurance: Patients with health insurance pay the membership fee each month on the same billing date. Our office does not bill individual health insurance plans directly. It is up to each patient to submit information to their insurance company to request reimbursement. Typically, the monthly membership fee is not reimbursed by insurance companies, therefore the patient pays this out-of-pocket.
